Pottawattamie County Releases 2026-2028 Community Health Improvement Plan

Pottawattamie County Public Health, in collaboration with healthcare providers, community
organizations, educators, residents, and regional partners, has released the 2026–2028 Community Health
Improvement Plan (CHIP), a three-year roadmap designed to improve the health and well-being of
Pottawattamie County residents.

Developed using findings from the 2024 Community Health Needs Assessment and guided by extensive
community input, the plan identifies three priority areas where collaborative action can have the greatest
impact: Social Determinants of Health, Access to Health Services, and Mental Health.

The plan outlines measurable goals and evidence-based strategies aimed at reducing barriers to care,
strengthening access to essential resources, and improving health outcomes across Pottawattamie County.

Maria Sieck, Administrator of Pottawattamie County Public Health, said “The Community Health Improvement Plan represents the collective voice of our community.” She added, “No single organization can improve community health alone. This plan provides a shared direction that allows public agencies, healthcare providers,
nonprofits, businesses, schools, and residents to work together toward common goals.”

The 2026–2028 CHIP builds on regional collaboration while addressing the unique needs of Pottawattamie
County and serves as a framework for local initiatives, partnerships, and community investments through
2028. Community partners will work toward the plan’s goals over the next three years, with future priorities
informed by a reevaluation towards the end of the implementation period.
